These materials can be used for collecting information from real people (classmates, parents, or others) or collecting information from classmates pretending to be the people depicted on the provided Character Preference Cards, including that character’s personal preferences as shown on the card.
The key difference between the authentic survey forms and the survey forms for other survey activity types, is that the names of the people being surveyed are not already filled in, so the survey-takers will need to collect the names of the people they survey and write them into the provided spaces on their survey forms.

Each survey form features rows for survey entries, with lined writing spaces in the left column to write surveyee’s names.
Simply print one for each student and bring them to class. No cutting or laminating required.

The following Character Preference Cards printables are available if an instructors wishes to have surveyees pretend to be the characters on the cards, rather than answering the survey questions based on their own personal preferences. This will generally result in a wider variety in the survey answers.
Character Preference Cards
There are 18 cards available. However, if there are more than 18 cards needed, more than one participants can have an identical copy of the same card. If this is done, however, surveyees should use their own names (or other fictional names, chosen for them by an instructor), instead of the names on the cards, to avoid 100% identical entries on the survey forms.
These cards are the exact same cards that will be used by the Weather Preferences Deduction Challenge activity (currently in development). Preparing them for this activity will mean that they are also ready for use with the Deduction Challenge activity (which practices the same vocabulary words) with limited additional materials prep-work required.
Laminating the cards is recommended, as it will give the cards long-term durability, even if used frequently.

Printing a unique pattern on the back of each set of the Character Preference Cards available above makes sorting easier and also provides a clean professional look.

Character Cards Based Surveys
These materials make it easier to ensure that learners practicing survey-taking will get a wide range of responses from the classmates they interview, as well as providing phonics decoding/encoding practice. Each person being surveyed will pretend to be the character depicted on a Character Preference Card and use that character’s name and communicate that character’s personal preferences.
However, unlike the Authentic Surveys version, the provided Survey-Taking Sheets already have a selection of the activity’s 18 characters listed on them, so participants will have to seek out specific people (who are playing the role of those characters).

Character Preference Cards
One Character Preference Card will be needed for each learner participating as a surveyee.

Instead of having learners interview a variety of people, with these materials, the students in a classroom can take turns using verbal communication in English to collecting information from a teacher (or other student) based on the SurveyInfo Table.

Information Trading Surveys
These materials can be used to practice asking and answering questions about different people’s personal preferences. Since each participant’s Information Trading Survey Sheet has a different mix of the activity’s collective personal preference data (about 18 characters) all of the participants will need to both ask and answer questions in order to complete their own Survey Sheet and help others complete theirs.

Choose a set of Survey Forms based on group size.
Always use all of Survey Forms labeled with a letter. This will ensure that at least one activity participant will have each item of survey information. The + Survey Forms are extras that can be used once all of the letter-labeled Survey Forms have already been distributed to activity participants.
Although these Survey Forms can be printed out and used once, if laminated, they can be written on with whiteboard markers. After completing the activity, the writing can be erased and the Survey Forms can be reused with other students later again and again.
